What is MLTC?
Managed Long Term Care (MLTC) is a New York Medicaid program model that helps eligible people who need long-term services and supports — often including personal care or home attendant services — receive them through a managed care plan. Rules, eligibility, and covered hours are determined by the plan and the state, not by any single agency’s marketing page.
If a hospital social worker or friend says “get on MLTC,” they usually mean: explore whether your loved one qualifies for Medicaid-funded long-term care through an MLTC plan, then work with the plan on an authorized care plan.
What families should ask
- Is my loved one already enrolled in an MLTC plan — and which one?
- Has an assessment already been done, and what hours were authorized?
- Who is the care manager, and how do we request a change?
- Which home care agencies are in-network for this plan?
- What happens during hospital discharge if authorization is not finalized?
